About OnTrack Tasmania 

OnTrack Tasmania is an NDIS provider whose purpose is to enrich the lives of people with disabilities. At OnTrack we have a strong focus on social justice and support people living with mental health or social issues to live as independently as possible in the community. If further support is required, we refer to more suitable mainstream agencies and advocate on their behalf. We also support our participants to receive further advocacy and referrals will assist them in getting the support they need. Whether it is getting into housing, joining community groups or gaining access to employment, we are with them every step of the way.

About you

This is an exciting opportunity to lead the artistic vision of OnTrack Tasmania. OnTrack Tasmania are looking for a dynamic, positive, reliable, empathetic and empowering approach to supporting people with disabilities. 

Reporting to the Community Access Manager, you will be supporting people facing mental health challenges, social disadvantage and living with disabilities to reach their full potential in a non-judgemental and supportive environment. 

Key responsibilities for this role include, but are not limited to:

  • Follow policies and procedures of OnTrack Tasmania and Uphold the values of OnTrack Tasmania.
  • Maintain a safe and clean working environment by adhering to WH&S policies, procedures and guidelines and by working in a responsible manner to ensure the safety of oneself, other staff members, participants and visitors to Ontrack Tasmania properties.
  • Facilitate development of participant basic life skills and foster community participation.
  • Report issues of significance to management and maintain records of significant daily events concerning the house and individual participants in accordance with policies and procedures.
  • Maintain participant records, write daily case notes and update the communication book.
  • Ensure the collection of statistical information in accordance with program guidelines.
  • Complete accurate and detailed Progress Notes according to guidelines before shift end
  • Engage in hand overs with oncoming staff member.
  • Basic administrative tasks.
  • Monitor participants’ behaviours and general physical state
  • Maintain participant and organisation confidentiality including, but not limited to: residents’ medical/ social history information told in confidence.
  • Display respect, empathy and understanding towards participant and their families at all times.
  • Interact and communicate with other direct care team members in a manner that achieves a consistent, high-quality approach to service delivery.

 

To be successful in this role, the following essential criteria is required:

  • Ability to play the guitar and/or other musical instruments
  • Ability to teach another person how to play the guitar and/or other musical instruments
  • Must hold a relevant Certificate III, IV, Diploma or Tertiary qualification in Disability (or its equivalent).
  • Demonstrated knowledge and experience in working with NDIS participants with complex needs.
  • Current First Aid Certificate.
  • Ability to work with OnTrack Tasmania’s Mission and Values;
  • Completion of NDIS New Worker Orientation and Covid-19 Infection Control Module;
  • Meeting the NDIS Worker Screen requirements of a current National Criminal Record Check and Working with Vulnerable People Clearance;
  • Covid-19 Vaccination Certificate or Immunisation evidence required
