About OnTrack Tasmania 

OnTrack Tasmania is a NDIS provider whose purpose is to enrich the lives of people with disabilities. At OnTrack we have a strong focus on social justice and support people living with mental health or social issues to live as independently as possible in the community. If further support is required, we refer to more suitable mainstream agencies and advocate on their behalf. We also support our participants to receive further advocacy and referrals will assist them in getting the support they need. Whether it is getting into housing, joining community groups, or gaining access to employment, we are with them every step of the way.  

About you

  • Ability to learn and master the workings of the NDIS system  
  • Build rapport with a variety of stake holders
  • Represent the brand of OnTrack Tasmania in a variety of meetings  

Key responsibilities to be eligible for this role include, but are not limited to:    

  • Understand and provide appropriate support to Participants with low and high needs who demonstrate a range of challenging behaviours.   
  • Assisting Participants to identify preferred supports relating to daily living, community support, and empowerment.
  • Work under general guidance from the person seeking support, and their family, within clearly defined guidelines.
  • Ability to work in a Case Manager role.   
  • Perform administrative duties and maintain accurate case records  
  • Participate in case reviews to determine the needs and wellbeing of the Participant  
  • Attend Team Meetings and other meetings as required  
  • Accurately record daily activities  

To be successful in this role, the following essential criteria is required:

  • Certificate IV in Disability Work and/or extensive relevant industry experience.
  • Experience in the Human Services sector in Tasmania and knowledge of a broad range of disability types including Mental Health Illness.
  • High level organisational and demonstrated computer skills.
  • Ability to write reports in a timely manner.
  • Excellent verbal communication and interpersonal skills including the ability to negotiate effectively.
  • Excellent writing and presentation skills to produce high quality reports.
  • Ability to work with OnTrack Tasmania’s Mission and Values.
  • Understanding of and commitment to the principles of person-centred planning.
  • Understanding of and the ability to apply NDIS legislation, NDIS practise standards and requirements of the NDIS Quality and Safeguards Commission.
  • Current unencumbered Tasmanian driver’s licence.
  • Meeting the NDIS Worker Screen requirements of a current National Criminal Record Check and Working with Vulnerable People Clearance.
  • Completion of NDIS New Worker Orientation and Covid-19 Infection Control Module.
  • Current Assist clients with Medication Certificate.
  • Current First Aid Certificate.
  • Covid-19 Vaccine record.
