Participant Intake and Engagement Case Manager
OnTrack Tasmania is seeking a Participant Intake and Engagement Case Manager to oversee person-centred, high-quality services for people with disabilities. This role involves managing intake and engagement for independent, short-term, and medium-term accommodation while providing intensive, recovery-focused support for individuals with complex needs, such as those with a history of homelessness, addiction, and dual disabilities.
Key responsibilities include:
- Coordinating participant intake and engagement
- Monitoring participant wellbeing, progress, and support needs
- Ensuring compliance with NDIS standards, policies, and procedures
- Collaborating with teams to match participants with appropriate support staff
- Maintaining accurate documentation and reporting
- Supporting continuous improvement and maintaining audit readiness
